In order to reduce barriers to care, the Diabetes Clinic at Ward 81 has appointment slots each Friday morning from 8:30am-12:00pm dedicated to ED patients who have a high risk of being lost to follow-up.
Please place the referral in Epic (Ambulatory Referral to Friday Diabetes Clinic) and do the following:
- Instruct patient to arrive at 8:30am (or 30 minutes before their appointment time if they are given one)
- Print and provide the patient with the handout linked below, which includes a map to Ward 81 (handout links also included in Epic order)
- If there is no Hemoglobin A1c for the patient in the past 3 months, redraw in the ED
- Medication: start patient on metformin (500mg PO BID if GFR >45)
Do NOT refer patient to the Diabetes Clinic if they are out of network.
